Trials are ongoing. The FDA has not approved stem cells or exosomes for most of the uses being marketed. Medical tourism is growing, and not all locations adhere to rigorous safety standards. Consumers should be wary of false claims.

My success hinged on one factor: working with highly skilled providers who adhered to FDA-approved methods and based their practices on the literature.
Not all clinics meet that bar. Some promote unregulated therapies or exaggerate claims, exposing vulnerable patients to unnecessary risk. To benefit from the promise, patients need more than hope — they need knowledge and discernment.
The Core Promise of Stem Cell Therapy
Stem cell therapy leverages the body’s ability to repair damaged tissue, by introducing or stimulating stem cells to regenerate new cells in areas like joints, tendons, and cartilage.
Documented benefits include:
- Knee pain relief. Stem cell therapy for knees has reduced pain and delayed joint replacement.
- Tendon and ligament repair. Studies show improved healing and mobility when guided injections target specific tissues.
- Avoiding invasive surgeries. Many patients have avoided procedures like spinal fusion through carefully administered stem cell treatments.
Outcomes depend on the quality of the product, the skill of the practitioner, and compliance with regulatory standards. The success stories don’t tell the whole story.
Where It Gets Complicated
Regulatory maze
Stem cell therapies face a patchwork of regulations globally. In the U.S., the FDA classifies stem cells as either:
- Minimally manipulated (Section 361). Lower oversight, often autologous cells.
- Substantially manipulated (Section 351). Classified as drugs, requiring rigorous testing.
In other countries, stem cell products may be sold with minimal oversight, producing wide variation in safety and quality.
Verify your provider adheres to FDA or EMA guidelines and uses sterile, compliant techniques. The FDA has approved blood-forming stem cells from umbilical cord blood only, and only for patients with disorders of the blood. No exosome products are FDA-approved. The agency has published four final guidance documents framing how it plans to support and expedite development of regenerative medicine products including HCT/Ps.
Product quality
Not all stem cell products are equal. Factors affecting effectiveness:
- Cell lines. Freshly harvested or lab-expanded? How were they collected, processed, and stored? Did they retain potency?
- Expansion cycles. Cells expanded beyond 4–5 cycles may lose potency and increase adverse event risk.
- Concentration. Proper dosing is critical.
- Testing. Are donor cells rigorously tested before and after expansion?
Potential complications:
- Tumor formation. Rare but serious, and linked to improperly processed cells or lines still in trials.
- Immune reactions. Some products, including umbilical cord blood-derived cells, have triggered organ-rejection-like immune responses.
Practitioner expertise
Success of PRP or stem cell therapy depends heavily on injector skill. Questions to ask:
- Experience. How many cases like yours has the provider successfully treated?
- Imaging. Are procedures performed under ultrasound or c-arm guidance? Without visualization, the needle cannot be safely placed.
- Technique. Injection often involves threading the needle along fibers in precise directions — advanced training, dexterity, and reps.
A skilled practitioner combines anatomical knowledge, technical precision, and experience.
Ethics and Patient Safety
Informed consent
Patients must be educated about the risks and limits of stem cell therapy:
- Variability of success rates
- Potential side effects (infection, unintended cellular reactions)
- Treatment costs, which can be prohibitive
Accessibility and equity
High costs make stem cell therapy inaccessible to many. Whether these treatments should be considered a privilege or a right is an open ethical question.
The Insurance Coverage Problem
The most pressing ethical issue isn’t in the development of regenerative medicine — it’s in access. Despite their potential to reduce pain, restore function, and prevent invasive surgery, many regenerative therapies remain uncovered. Meanwhile, insurers routinely approve invasive surgeries that are often costlier long-term and carry greater patient risk.
The cost of inaccessibility
Surgery vs. regenerative therapies. Joint replacements and spinal fusions can cost tens of thousands of dollars, require lengthy recovery, and carry risks including infection, complications, and prolonged disability. PRP for tendon injuries and stem cell therapies may cost significantly less and often allow faster recovery with fewer complications.
Insurance priorities. Reluctance to cover regenerative treatments may perpetuate a system that funnels patients toward higher-risk, higher-cost procedures. Why are less invasive, potentially more effective therapies being overlooked?
Balancing Hope and Caution
Stem cell therapy holds real promise. Patients should approach it with informed caution. Some clinics offer seemingly attractive options that, without proper guidance, can produce poor outcomes or unnecessary risk.
Key considerations:
- Informed consent. Fully understand risks, benefits, and limits.
- Safe practice. Look for clinics that use sterile environments, visualized injections, and properly sourced products.
- Regulatory compliance. Treatments should align with FDA, EMA, or equivalent standards.

These products are approved for use in patients with disorders that affect the production of blood (i.e., the ‘hematopoietic’ system) but they are not approved for other uses.
Exosome products are also regulated by FDA. As a general matter, exosome products intended to treat diseases or conditions in humans require FDA approval. There are currently no FDA-approved exosome products.”
